I used a template to construct a form:
It displays correctly but the emailed response is a mish-mash of my form and the template questions. Can this be fixed?

Usually, problems concerning email alert templates can be fixed by deleting and recreating the template itself.
To delete, click Email Alerts > Notification > Delete Email.
To create a new notification, click Email Alerts > Add New Email > Notification > click Next; then follow and compelete the Email Alert Wizard
Save the form afterwards. Let us know if you're still having issues with this.
