I am in the process of writing an extensive form for an intake for a men's group. I believe I have figured out the Check Box Tool but I need to take it one step further. After a person checks off a box for something they have done I then need them to give me the month and year the last time they did that action. For instance...
When was the last time you ate apples? (This is is just a mock question.)
I can get the box, the _____/______ to show and the question but is there any way for the client to fill in the blanks to give me a date (month / year) they last did this behavior?
When was the last time you ate apples?

You may try using our Narrative Fields widget that allow you to show text and as to put Texboxes among the text so that your form users can enter information. Here it is: http://widgets.jotform.com/widget/narrative_fields
I think that is the tool that will allow you to get closer of you what you are looking for. Since I tried to apply a workaround using the form ful source to add some textboxes, but it will not work as the data entered in those boxes is not sent through the email lert:
