Is it possible to create a radio button that shows one label but inputs a different value?
I want to have a radio buttons that show descriptions in the options, but I just want a numeric value submitted in the data. For example: I want a button options to say
"many mistakes" (with a value of 1)
"3-4 mistakes" (with a value of 2)
"1-2 mistake" (with a value of 3
"No mistakes" (with a value of 4)
I want to create calculated fields based on these numeric values, but I want more explanation in the labels than just numbers. Is there any way to do this? Any widgets?

We do not have any direct way to assign values to the field options via the editor. It should be possible with downloaded form source code and custom scripting.
I think it is a good idea to have such a feature, so I am forwarding a feature request ticket to our Development Team. We can not provide any ETA at this time, but we will update this thread if we get any news.
