I tried few times to do so, but it still says that I am a Premium user, which I am not. Additonally most of the time my app is not showen on facebook! I am seriously considering giving up JotForm!

There is a little problem with your embedded form on Facebook, it has incorrect httpS URL, that is why the form is not being loaded:
