I have a form which was working fine but now is not. It is still submitting the information, but the formatting is all over the place and it seems to be adding all the answers in text on multiple occasions before the correct format. Difficult to describe but the attached screenshot shows what I mean. The one on the left is what it should look like, and was until a few days ago. The one on the right is what it looks like now. Would love any help anybody could provide on this.
Have you done any changes in your form's notification email alert recently? Upon checking your form's notification, I did not find any issue with that. The submission emails format should be displayed correctly.
I would suggest you to delete the notification email alert and add it again. Here is a guide to help you delete the notification: http://www.jotform.com/help/132-How-to-Delete-Notifications-Autoresponder
