How can I choose a question format that allows more than one choice?

I have a few questions for a survey that are not simple yes/no/maybe choices. For example, I want to know if they attended one or more of the past meetings. They could pick all 5 past events, but right now the form only allows them to choose one.

For multiple choices it is better to use check boxes field rather than radio buttons that more directed to one choice.
