Sent e-Mail does not contain Header or Subheader text. How do I make jotform send the Header and Subheader text?

The email report received does not include the Header or subheader data. I have tried to use html text to identify the subareas of my form, however, that text does not appear in the data sent via e-Mail either.  My form is layed out in sections with redundant questions. It is imperative that I have the Header/Subheader/HTML text to divide the sections of the report. Example:

    Restaurant 1

          Question 1

          Question 2

          Question 3

    Restaurant 2

          Question 1

          Question 2

          Question 3

    Restaurant 3

          Question 1

          Question 2

          Question 3

If the customer only rates 1 or 2 of the restaurants, How does the e-Mail reviewer know which restaurant/s was/were rated if the e-Mail does not contain the Header/Subheader/HTML text "Restaurant #".

If 2 restaurants were rated, Without the Header/Subheader/HTML text, the e-Mail notification looks like this:

    Question 1: text

    Question 2: text

    Question 3: text

    Question 1: text

    Question 2: text

    Question 3: text

Now: Which 2 restaurants were rated?

I do not have the form posted on my website since the e-Mail notification is useless without this information.

Here is the link to the form on the jobform site: http://form.jotform.com/form/11954054836

    Hi kntadmin,

Thank you for your message. You can manually add the headings and sub-headings in the template of the email notification itself.

The Email Notification Wizard is like a huge Free Text (HTML) field complete with its own toolbar, wherein you can add images and text and format it according to your specific needs (i.e. bold, italics, change text color or font face, etc).

To go to the Email Notification Wizard, please click the Email Alerts button under the Setup & Embed tab of  your toolbar. Then click the notification that you want to edit. Once you see the Free Text (HTML) Area, manually add the headings and subheadings. You can use the built-in toolbar to modify the text appearance. Then save the notification.

We hope you find this information useful. Let us know if you need further assistance.

    Thank you for using JotForm!

O.K... Fixed... Had to edit in text mode. Now it retains my configuration. What a bear...
