Card form: Questions are shifted or not showing

    Hi, thank you for contacting support.

    Are you perhaps referring to the form https://www.jotform.com/form/212908675782872?

    As per checking, there seem to be several incomplete conditions. Could you please try to fix these conditions first and let us know if you can't still submit the form?

    Ok - I have deleted all those conditions (they must have been imbedded in the template I used, as I did not create them).

    However, still the same issue?

    I checked the privacy settings, seems that your form has been set as private.

    And only the person who is allowed to access it will be able to view it.

    You need to change privacy settings in order for your users to view it.

    The Issue was still occurring when the form was public.


    I have re-selected 'Public'.


    However, when you get to the second last question - Should be Name and then Date - the fields do not even appear on screen:

    Hello,

    I apologize for the inconvenience. We are working on the issue.

    You can follow these steps to continue to use your form.

    1- Click on the form designer.

    2- Select any theme on the color tabs.

    3- Duplicate the hidden box field.

    4- Delete the first hidden box field.
