I use a drop down box for the recipients to choose their name. Previously, their name would appear, in addition to the form name. Somehow, I lost everything, and had to use a template to recreate my form. I've been able to do everything i need, but get their name on the subject line of the return email. Can you offer assistance?

I believe that you are referring to the Notification email, correct?
To insert the respondent's Name into the Notification email subject line, you can click on the NAME form field and add it to the Email Subject line as shown below:
Is this what you were referring to?
