Multiple Conditions on One Radio Button Questions

Hello,

I am trying to use multiple conditions on a radio button and only the first condition is working. When a radio buton is selected, we want only the corrsponding questions to be available. When selecting the first radio button, the appropriate questions appear and the rest dissapear due to the condition I set. However, after selecting the second radio button, it is not following the conditions I set. I have not set the remaining conditions because I can not get the second one to work. 

Can you please help me with this? Thank you for your help.

The issue is caused by conflicts between Show and Hide conditional actions. In our system, the corresponding Hide conditions are not required if you use Show conditions. In other words, we can use Show conditions, and it will automatically hide other options.

For example, on your form we will need to set the Show actions for each Radio Button option:

Thank you for your help. That seems to have worked except we are still getting come extra boxes outside of the conditions set up. Such as extra "Why Does this nominee deserve this award" text areas that shouldnt be there when we select an answer to the radio buttons.

Any advice on how to remove those?

It looks like that one of 'Why does this nominee deserve this award?:' questions was used in a wrong order.
