Can we count on PHP being supported FOREVER?

I very much like the PHP integration.  Of course, it is a lot of work to integrate a form with dense PHP code.

How confident should I be that PHP support will be something that sticks around a very, very long time?

I suppose it will be forever available, since JotForm also runs on PHP.

I assume your concern is that similar to the javascript/script being blocked when embedded within the form due to security measures applied in the form.

Its different with PHP though, because you can use it outside the form and its server-side. So the consequence of using PHP will on the user's end.
