I use this form in clinical assessments. I want to keep clients anonymous. Do prospective clients need to sign up with JF or register?

Hello there Brian!
I understand that you respect your prospective clients privacy and would like them to be anonymous when filling out the form.
To do this, you just need to set your form to "Public". To do it, Go to Publish > Quick Share > Settings > set it to Public

Hello Brian,
Settings into Private Form would require your client to sign up and create their own Jotform account. However, as a workaround, you may instead make your form into a Public Form then set a Password to share with your clients.This way, your client will not need to sign up.
