Help pls.
As I understand the Spreadsheet to Form column should auto-fill all the fields after it as long as the field labels and column header names match. I've tried different ways but the widget won't fill 2 of fields I have. Can you help please. I can't see what's wrong anymore. I need fresh eyes to look at my form. Also note, that I have the same set-up with other forms and they work perfectly fine.

Yes your understanding is correct. The question label of your form and the column label of your Excel spreadsheet should match. Once the user input the unique code in the widget, it will fetch the existing row data from the Excel file and populate in the form field.
I would suggest you to please share a screenshot of the uploaded Excel file columns and we will take a look.

Thanks for your patience.
I've checked your form and found that there were two issues causing the widget not to populate fields as expected:
1) Your form fields names missed the # sign at the end, so I've added it:
2) On the spreadsheet file there were additional space characters after the column names, so I also removed them and saved them:
I've updated the spreadsheet file on your form and it should populate fields as expected now.

We would like to apologize for any inconvenience. I have checked the issue you are having and it seems that the issue is with the label of the TRW# and GVW# fields on your form.
Updating the label of the fields to remove the extra space at the end should still be fine since your form does not have any submissions yet.
You would also need to update the column headers on the uploaded spreadsheet for TRW# and GVW#.
Please upload the updated version of the spreadsheet and test your form again after.
