Hello Art - I can see that the form embedded on your website is the old form which has been tagged as inactive/dormant.
Since you mentioned you already created a new form, we suggest it best to remove that form on your website and replace it with the new one by re-embedding. You should have an access to the backend of your site to modify it. Here's a guide on https://www.jotform.com/help/34-embedding-a-form-to-a-web-page/

Hi JohnRex,
I did replace the embedding script with the new one but I'm still getting the same message of Form not found and This form is in restore mode, please try again later.

Hi, Art
It looks like you have used the wrong embed code to embed the form. You should be using this embed code:
<script type="text/javascript" src="https://form.jotform.com/jsform/213095634610047"></script>Please let us know if changing this worked and if not we can try embedding using the Iframe method.
Hope to hear from you soon. -

No that didn't work, so let try the iframe method.

I have checked that your site is still using the old form:

I can see you are using WordPress. Please ensure you are editing the correct page. As I can see, it should be the homepage.
