I thought I set up the google docs integration successfully, but the data is not being transferred. Please help me figure out what I did incorrectly.

Hi,
I made a little changes to this clone version of your form: http://www.jotformpro.com/form/40795159298975. Please clone it back by following this guide: How to clone an existing form from a URL and integrate Google Spreadsheet. See if the data still don't get submitted to Google Spreadsheet.
Or you may do the same changes below and re-integrate the form:
a. remove colon symbol from the field "Please explain your response here:"
b. remove colon symbol from the field "A recognition program will positively impact:"
c. remove three dots or fullstop symbol from the field "Successful managers should recognize their employees at least..."
d. remove three dots or fullstop symbol from the field "Vote on any of these..."
