I think the sample you showed is doable if you will use Widgets Button Checkboxes or Button Radios.

Here is how you can create the blocks with predefined values.

• Go to Forms and click Build > Add Form Element.

• Go to the Widgets tab and type Button Checkboxes or Button Radios and add it to the form.
