How to create a form for Review/Signature and then Approval

I need a form that:

1. A supervisor will complete.
2. Email to a student. (how to I make this easy to change for other students)
3. The student will review/sign and if necessary make comments
4. Return to Supervisor for final approval and signature.

Is it possible to create one that when the student receives it that it shows up in their email as "Review and Sign). I've attempted to create one but when I send it to the student it shows up as "edit the submission".

The Approval Flow goes like the following:

I did not add a Deny flow. Add it if you wish to have it in the flow.

So, the Supervisors fill and submit the form, the student receives the approval email and clicks the Approve button, and signs it. The Supervisor receives an Approval, click and sign and that's it!

Could you set this up so when I want to upload it the approval process you added included? When I uploaded it, the process was not available.

Could you set it up on the clone’s form so I can upload it and make changes?

I have checked your form but I can't seem to find an upload field.

Can you please share more information on the flow of the form ?

Do you want to trigger an approval once an file is uploaded or when the form is edited?
