Fillable PDF not loading correctly

Rather than an online form, I want to create a fillable PDF. I have my form the way I want it, but when I make it a fillable PDF, it changes the order of a few questions at the end and doesn't look right at all. I've edited and edited, but don't know how to correct this. Help please.
