1. Can the app show our clients their data entry like pictures and weigh-ins?
2. Can we hyperlink out?
3. Can they add pics and be able to see it for themselves

How do I create clickable images or tiles that go to another page? Similar to the video

It is possible, using the Image widget, you are able to upload photos and attach a URL to it:

Anyway to keep links and image links in the app or embedded?

Unfortunately, the Jotform app builder will not be able to work in the way you have shown on your video.

You can only add elements on the app, but the image field has only one option to hyperlink it as shown by my colleague above. There is also an image gallery but it does not have an option to hyperlink images.

Your requirement can be only achieved using a custom app development setup.
