HIPAA Compliancy Forms

    Hi there!

    We are looking to make forms HIPAA compliant and wanted to know if you all would be able to assist with something like that. Is there a way I can speak to a sales rep on the phone to discuss the nuances of your services?

    Hello,

    Phone support is only available to Enterprise users, but we can answer your questions that you have here.

    If you would like to have a HIPAA enabled account for your forms you will need a Silver or higher subscription. Once you do you'll have the option to enable HIPAA on your account which will migrate everything over to the HIPAA servers. At the end of the setup you'll also sign the BAA.

    Hi Parker,

    Is there a limit to how many forms we can have? Additionally, how many questions are we able to have on each form? We need multiple types of patient forms for our pharmacy. Do you all support integrations such as MailChimp or Shopify?

    Hello Maryam - The number of forms that can be created in an account depends on the subscription. For example, if you're on a FREE one, you can only create up to 5 forms. While, if you're on a Bronze plan, you can create up to 25 forms.

    As for the number of questions, you can add as many fields as you want but please take into consideration that the number of form fields may affect the loading performance of the form - especially if you used widgets.

    Hi there,

    Just to clarify - in order to get the HIPAA compliance - we’d need to purchase the Silver or gold package? So Bronze or Free isn’t really an option for us based on what we’re looking for correct?

    Hello,

    Yes that's correct HIPAA is only available for Silver and Gold plans.

    Could you please tell me more about how your extension works? Would we have to use a plug in or is there a code we can integrate? Additionally, do you have any plug ins for Wordpress that are HIPAA compliant?

    How do we view the patient data once it is collected in the forms? How many users can you have on each level of account to view the forms? Can you upgrade or downgrade user level at any time and can you switch from month by month to yearly payment at any time?

    Hi Maryam,

    How do we view the patient data once it is collected in the forms?

    Due to HIPAA privacy regulations you will need to login to your Jotform account to view/read the submissions. There is no other way for this when HIPAA is enabled for the account. You will not received any form submission information via email.

    How many users can you have on each level of account to view the forms?

    Please note that one account can have only one user. If you want your client to be able to view submissions, you can share tables or add a report in form and share the password protected report URL to your client. Allowing other users to be able to access submissions is not possible in a HIPAA-compliant account. If you upgrade your account to HIPAA compliance, only account owner with login id and password will be able to view submission.

    Can you upgrade or downgrade user level at any time?

    You may create a new account or upgrade your existing Jotform account to HIPAA compliance. You would be able to go back to the non-HIPAA format. However, we recommend downloading your data before you disable the HIPAA compliance.

    Can you switch from month by month to yearly payment at any time?

    You can choose to subscribe to the paid plans either on a monthly subscription or a yearly subscription, and the subscription is automatically renewed until you decide to cancel. To see our pricing and what is included in each plan, please see our pricing page here:

    Just to clarify can we start off with a monthly subscription and then halfway decide we want to upgrade to yearly? Or are we locked into that subscription plan until it expires?

    Hello,

    You can start off with a monthly subscription and switch to an annual one. It will extend the contract for a full year.


    Now let's imagine that 6 months later you decide that for some reason you do not need the service anymore, you can cancel, let us know and we will refund the unused months.
