I use JotForm to obtain client information, and for some reason my survey is not showing up on my account. It shows three surveys I made in May, but I cannot access my most important survey, which is found here:http://form.jotform.com/form/11665047148
Why is that not showing up? Is it under a separate account or something I need to log into?

Apparently, that form is under a guest account, meaning you were not logged in to your registered account, philpallen, when you created that form.
Guest accounts rely on browser cookies, and the moment something happened to these cookies (cleared, overriden, etc), you will lose access to this form and the submissions made to it.
The only way to restore access to the form and the submission data is to register an account using the same email address entered in the form's email alerts. As I can see though, this form does not have a notification configured, so there's no way to have the data restored.
You can instead have this form cloned/imported to your registered account, but the data cannot be carried over. Click here to learn how to import an external form to your account.
