When I add this widget my rows and columns (including the add row button) are not visable until after I refresh the page. This happens both when the form is embeded or accessed directly via the url.

So, probably it's something related to the browser. Can you please try using a different browser? or kindly Clear your browser, that may help.

That is the correct form, I have confirmed it works in chrome but does not display in IE or firefox. Can you reccomend a fix for this?

I tested your form in FireFox, but the it worked perfectly, the widget loaded with no issues:
I also tested the form in IE, but I was not able to reproduce the issue.
So, it is most likely this issue is related to the version of the browsers you are using. Can you please let us know the versions of the browser you are using? I used FireFox 28.0 and IE 10.
We recommend our users to use the updated version of the browsers to create and test their forms.

The issue is related to when this widget gets used in conjunction with the show/hide conditonal statements.
Please use the type of visit - service call option to show the Open issues field. I have confirmed I am viewing this in Firefox v28 and IE ver 11.

Thanks for the clarification.
Now, I confirm that the issue also happens from my side while using FireFox and in IE 11. I conclude that this issue happens just wnen widget loads while being hidden.
So, I have forwarded this issue to our next level support so that they can help us to resolve thei ssue as soon as possible.
We will update you via this thread once it is resolved.

A temporary fix is just applied in order to cover most of the problems related to Firefox and IE behaviour difference. We will make a permanent fix very soon. I would like to thank you very much for pointing out this issue.
Please let us know if it still has troubles.
