NO confirmation received after submitting form

    Good afternoon,

    I submitted a completed jotform for our company but never received any confirmation. The information was to be completed by today and I submitted mine on Nov 20th. However, when I try to access the form I'm getting an error that reports "The gift product offer has ended. Thanks for participating!" despite of the last day of being able to do so being today.

    How would I know that this was successfully processed for https://form.jotform.com/213133757779163

    Hi,

    Apologize for the inconveince.

    The form is disabled because the owner of the form has set it that way. I looked into our email logs and looks like jrouse@smgnj.com has no submission. Can you please provide us with the email address you used for the form or any details you entered into the form so we can look into it and make sure the submission went through?
