    If you can explain further the usage of the form, we would be able to present you an examples. Or, please take a look to our form template gallery: http://www.jotform.com/form-templates/. You should be able to browse forms by its type and/or industry.

    We are looking for a 3 part form that we can write down sales leads for boats. Names, address, phone number, cell, email, boat information, what brought you in, trade information etc

    Hi Kim,

    Did you check our form templates gallery? You should be able to find a similar form there. You can clone a form and modify it to match your preference.

    Or, if you can present us an example form from the other sources, please do and we'll check and try to come up for an example.

    I would also suggest checking this guides: How to Create Your First Web Form and Quick Overview of Form Fields. These guides should help you create the form and know our available fields.
