Hi,
I cloned the form and was able to reproduce the issue.
It seems that the current Theme causes issues with the logo.
Could you try changing the Theme for the form?
This should make the logo display correctly.
If that would not help, please let us know. -

Thank you for your prompt reply.
I opted for removing the logo element and using a Header element.
This seemed to work as well.
