Change Input Table's input from Radio buttons to Textbox

Hi There,

I need help with inserting a table. I want a table where people can fill in the answers in blank spaces.

When I insert an input table, it has dots for the person to fill or click. Instead I want it to be a blank space so that the person can fill out their own answer. How do I make this happen?

I see such a table was created when Jot Form converted my pdf, but I don't know how to do it myself.
