Why am I not getting submission?

Every time I fill out a form your app just takes me off the form. I recorded the screen when I was filling it out if you need proof it was not caused by the user(myself).I pressed submit twice and jotform did not notify me of the submission.please keep in mind that I was filling out my own form to see if it was working correctly.

Could you upload the video on Google Drive and share the link with us?
