Hi,
I made my form and everything is great except for a few questions that are blank after it has been submitted. I know there was information in the text area field when it was submitted, it is even a requirement.
I have it set up as a conditional question that uses a radio input to test the condition. If ture then it will give a text area to explain more in. The info is in there when I hit submit, but when I get the response back to my email the questions are left blank.
I do have other questions that are exactly the same as the ones that are left blank and they work just fine. I have tried deleting the questions and the conditional statments, but it keeps doing it to only those questions. Why?

Also it leaves off this question: Do you need to cover a dependent? & leaves this next question blank: Names, Date of Births & Sex:

You have changed the email notification template, so that the template is no longer auto-updating itself. It is possible that the form was updated and some field {tags} are out of date now.
Can you please remove your current email notification, and then re-add a fresh one?
Please give it a try to see if that makes any difference.
