Did I configure it incorrectly?
I chose
NAME
NO REPLY JOTFORM
EMAILfor the reply stuff
BUT when it came thru, it had this at the top:
=?UTF-8?B??= <noreply@jotform.com>
HOW TO FIX?

After looking at the Email Alert on the form embedded on the URL you posted above. I do see it is missing the Sender Name:
Kindly add a Sender Name to the Email Alert. And perform a test submission.
