I tried to embed contact form on my site and i get error 404 page. pls advice

I'm able to see the form in your webpage http://caisosteelband.typepad.com/caiso/contact-us.html with no problem. So it looks like you were able to sort the problem out youserlf.

I don't think forms work. i'm tried to test it. no submitting

I am also not able to replicate the issue you are having. I am able to open your webpage without any error message and it does display the embedded form. Please check the screenshot below:
The issue seems to be that, I was unable to submit your form as it redirects to someother page asking for email address. Upon taking a closer look at your web page, I found that you have used your form's source code to embed your form. Is there any requirement to do so? I was unable to find any custom code you have added.

I would suggest you to embed your form using its iFrame embed code and that should solve your problem. The following guide should help you grab iFrame code: https://www.jotform.com/help/148-Getting-the-Form-iFrame-Code
