This form allows suppliers to upload inspection results but one supplier says he cannot do it.
The filws have the old extension of .xls and .pdf, I have been able to do it using IE9 but the supplier is running IE10, could his be the problem?

I cloned and tested your form using IE10 and I'm able to upload and submit the form successfully. Kindly ask your supplier to reset the browser to factory default and see if that helps.
Here's how you can reset your browser to factory default.
1. Click on internet options
2. Click on Advanced
3. Click on Reset
4. Choose Delete personal Settings and click on Reset
5. Click on Close once done
