When I try to create Custom URLs, sometimes the domain name is jotformpro.com, sometimes it's form.jotform.us, and sometimes it's jotform.us. How can I get Jotform to assign a consistent domain name? I'm trying to create a set of forms with a consistent naming convention; it's going to be very confusing for users if they have to enter in a different domain name each time.

We have many, many, domains to help cover and optimize connections to our platform from various sections across the globe. At first you are assigned to the server which is closest to you based on your location and ip but if you become a subscriber it will change to jotformpro.com as you've seen.
I checked in our system and the majority of your forms are currently listed under " http://www.jotformpro.com " and several others are under " Http://www.jotform.us ". So I would recommend using Http://www.jotformpro.com to log into your account from now on and that should help you to keep it more consistant.
Also, It is generally a good idea to pay attention to where you are logged into your account from in the browser address bar and also where you go while navigating our website since sometimes clicking on other links on the site can convert your page such as clicking on the forum itself where it converts you over to jotform.com in example. The same can happen with cloned forms if using a form from a different server that has been copied to your own and you visit it that could change it too.
Additionally, You can easily manually change these to work though sometimes that is not the case where if so then it will pop-up with a redirection url telling you the correct link to use.
