Have to refresh to view form

Hi, we use Jot Form on two websites, clearchoicephotobooth.com and payclearchoice.com.  Sometimes on the payclearchoice.com and always on the mobile version of the forms, the page will show up with just the title.  But if I refresh it, the form shows up completely.  These are payment forms, so it is very important that they show up every time.  Do you know what the problem could be?  We appreciate any help in this matter.

Thank you, I did the iframes for all the pages with forms and it seems to work.
