I would suggest to try changing your Sender Email Address from firstname.lastname@example.org to email@example.com to check if you do receive emails through that standard configuration.

I edit the form and run an e-mail test which is successful. Still not sure why I'm not getting e-mail to my Yahoo account since April 4th. Other e-mail is coming to me from other sources and the form database is registering new submissions. The submissions are the only way I know people are using the form.
JotForm did have issue with Yahoo mails few days back, but it had been rectified since.
So, I am not really sure if it still is the issue on your case.
If you need to really use your Yahoo mail, and have no other e-mail option available, you may try using SMTP mail via Mandrill service. Refer to this guide
