I can't login to my account and when I click on "forgot password" and enter my email it says it's not a registered email. I only have two emails that I use and they are BStewart@MyMidwestFamilyWellness.com and FinkChiroMktg@gmail.com and neither of them are recognized. Please help!

Unfortunately, our records shows no account for both the email address given. Do you have a form embedded somewhere were we can check? We should be able to trace the account using the form ID.

Also, we do need the URL of the standalone form or the location where the form has been embedded online. As we do not have any account under the email addresses you have provided. But by accessing the form we may be able to rtrieve the correct account.
