Your form has been marked as suspicious. You probably cloned a private URL form since when cloning a private URL it does clone the login section on the upper right and our anti phishing tool doesn't allow any login forms special forms that contains username and password.
I have deleted the form that causing the issue. Try cloning the form again or give the form ID so we can clone if for you.
