e.g., How can I create a successful survey form?

    Patients will submit an enrollment form but I am not getting notification that they did.  How do I fix this?

    Hi, I checked the email address used for submission notifications (kcaldrich@yahoo.com), and it is not in our bouce list. Have you checked your spam folder?. You may white-list our domain names if the issue persists. (We use Amazon SES method for "noreply@jotform.com" sender.)
