Im writing to you to query an issue we had with the submissions.
We have a form set up along with a notification which goes to a dedicated email address which has worked great for us in the past.
It has now come to our attention that we have had submissions come through to our email but do not appear on the spreadsheet.
Missing submissions range from the 25.06.2011 to 21.07.2011.
If there was any issues in this period, could you please let me know.

Thanks for bringing this to our attention. I was able to confirm the issue you described. I shall hand it over to our Tier 2 Support for further investigation and corrective action. Rest assured that we'll update you once findings become available.

We have not had any issue during this period. Somebody who has access to your account or computer might have deleted the entries on the submissions page.

Thank you for getting back to me but, no that isnt the case as we keep all our submissions as a form of cross referencing.
I guess we will now have to keep a close eye out on the email notification along with the submissions form just in case it happens again.
