Problems with form information

    Hi -

    1. In my form Portland Nursery Questionnaire I have sections that require a checkbox answer. These answers are not showing up in the 'results' that are sent to me.
    2. Can the results come to me in the form of a pdf.. I have made a pdf but can't figure out how to indicate it should be emailed to me.
    3. When i get the results, they appear in an email from Jotform vs my company. It appears with the info at the bottom 'now create a jotform of your own'
    4. I created the form before we upgraded - does that have anything to do with it?

    1- To answer the first question:

    Open the Email notification option and go to the ADVANCED tab,

    then Disable the Hide Empty Fields option

    2- To answer the second question

    Please enable the PDF attachment option

    3- I have removed the bottom in the email body.

    4- The only thing that cannot be removed is the Jotform branding at the bottom of the email body, but once you upgrade, it can be removed.

    i'm getting frustrated. So, I did what you said and things started working. Now they aren't working again. i've checked the settings. I've cloned the form to start over.

    2 problems:

    1. the select fields are not showing up in the email.
    2. the subject for the email is filling in with a form answer

    1. I would recommend you to delete the current email notification and re-create it
    2. The subject is set up to show the Form Title and the Full Name , every time someone fills the form out, it show their name, isn't it the desired functionality you were expecting to? You can simply delete or edit it and put the desired subject at the email setting

    Hallelujah -- that fixed things. Also, the other issue i was having was not the subject (which was fine) but rather the 'from' field showing up with an answer from the form.. it worked ok now. I'm not sure how to change it from coming from 'Jotform' as a notification, but i was able to change it for the autoresponder.
