when users and potential new members log on to the guest registry an oops message occurs. we would like to know why that is happenning and how to correct it.Page URL:

Your form was created under the old Jotform system. You would need to migrate your forms over to the new Jotform to make them work.
Please log in to http://v2.jotform.com and then choose the migrate option to make your forms v3-compatible.
