Conditional Logic Not Working

Hello. I have conditional logic on the following form that is supposed to show/hide the "select a product" field. It is not currently working. I have manually hidden most of the fields to not confuse my customers while troubleshooting. But normally, this works as expected. "Select a product" fields are hidden until you select a size. But today it is broken.

I checked this on my end for the first question and so far it's working. I started looking at this more and a couple more conditions in I found a duplicated condition problem and also a problem with the fact that you have a lot of conditions so it's created a conflict on your form. 

After I checked and fixed that plus removed your hidden fields I found it is working. Here's the clone of your form I was using to test this problem with: http://form.jotform.us/form/41116123519142

The second condition had two rules so this was cancelling it out.
