So, it is the Preview mode of the form in Jotform the one that is not workin, right? Are you getting any error? Please provide more dteails about the issue so that we can provide you the proper assistance.

I checked the form on your page and I can see it's working fine.
It does not freeze at all when I tried to fill the fields. Is this already resolved? Or was your new created form worked?
