progress bar widget is showing codes in the form

Hello Wonderful people. Hope everybody is doing alright.

I Need help with an unwanted code displaying on my published survey form. I have attached a screen shot showing the code.

Please note this was a test embed on Google sites.

Is this an error from my end? and how can I fix it?

I have updated the widget and it seems the issue is resolved now.
