To whom it may concern,
I registered to get a COVID test for Thursday (tomorrow), but Ally Health via Jotform sent me my "COVID Test results" already. This is very alarming, and I may need to report this incident to the Maryland Department of Health.
Please resolve this issue as soon as possible.

I understand that how alarming that situation is.
If you're not familiar with Jotform, it's a tool for creating web forms. Many of our users use Jotform to generate order forms, survey forms, quiz forms, and other types of forms.
I understand you're attempting to resolve an issue with a business/user. If you received a confirmation email after submitting the form, you may utilize the Reply-to option in that email to contact the form owner directly and express your concern to him or her.
