Creating a form with toggle button

I am looking for someone to build a form. I am needing this form to be formatted as a checklist with items for a user to mark. Based off the user's entry the forms should have responses to answer. At the end I need the form to generate a statement that can be copied and pasted into a document. An example of what I am needing built is in the link to the video below.
