I need an order form/purchase order without PayPal connection

I need to make an order form to collect orders, qty and calculate the total, but I cannot find a way to do it without going through PayPal or Authorize.net.

Our orders need to be forwarded to a distributor.

Is there a template to start with, or what do you suggest?

Fortunately, we have a Purchase Order tool under Payment Tools. It does not forward users to any payment gateways, so it should work in your case.
