How do I edit the From and Subject lines of the Embedded Form I send via email?

After I tested sending the embedded form via email, I noticed the From email was noreply@jotmail and there was no subject line.  This may send my form to junk and spam boxes, which I want to avoid. 

How do I edit these fields?
