How can someone select more than one option on a form?

I have created a list with buttons indicating choices, and I would like for people to be able to make more than one choice. Is there a way I can enable that in the construction of the form? Or is there an instruction I can give them that will allow them to make more than one choice?

    Thanks!

    Hello zollaa6,

I did check your form and found that you are radio buttons. Please be noted that radio buttons will only allow your users to make only one selection. If you want them to select multiple options, you should replace the radio buttons with check box questions. Please check the screenshot below:

Please take a look at the following form and see if this is what you want to achieve:  http://form.jotformpro.com/form/41174599568975? Feel free to clone this form for a closer look.
