For some reason my most recent couple of forms don't work on a cell phone. attached is a screen shot. Do i need to change the formatting?

What is the form URL in question please? OR, share to us the webpage where the form is located so we can check.

Usually, that error appears on redirect page when you provided the wrong keys in the captcha field of the form. Sometimes, a conflict would also cause this so please let us know the webpage URL to where the form is embedded so we can check.
You can also try re-embedding your form using our iFrame Method: http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code so to fix the possible conflicts the causes this error.
