I have sent this form to various people and they can not put a date in. I then tried it and I also can not put a date. This has been an issue for many parents in the past.
Since the date can not be filled in, you can not complete the form. As I said. I have sent it to 4 four people and all four can not input a date.

After further checking and testing, I would kindly advise you to turn off the "Lite Mode" in the "Date Picker" field's "Options" as shown below.
